If you’re making plans for international travel, you can make an appointment to apply for a U.S. passport at the following Library branches: Corporate Parkway, 1200 Corporate Parkway, Wentzville Kathryn Linnemann, 2323 Elm Street, St. Charles Middendorf-Kredell, 2750 Highway K, O’Fallon Spencer Road, 427 Spencer Road, St. Peters
These branches are designated U.S. Department of State Passport Acceptance Facilities. Appointments are required. Afternoon, evening, and weekend times are available.

The passport acceptance process takes approximately 30 minutes per application so please plan accordingly. An additional 30 minutes per application will be required for hand carry services. |

Fees for a Passport Book
Fees for a Passport Card A passport card may be used for international travel by land or sea between the United States and Canada, Mexico, the Caribbean or Bermuda. It is not valid for air travel.
Execution Fees The Library must collect an execution fee for each passport application processed.

Passport Checklist In order to apply for a passport book or card, please read “The Application for U.S. Passport or Registration” (Form DS-11). Copies of this form are available at the Library or can be downloaded from the State Department’s passport page. Applicants should complete the following checklist. Missing information will result in a delay in your passport processing time.
☐ Complete DS-11 passport application. This form should be completed in black ink and have no white-out or scribbling. Do not sign the application until meeting with our passport services staff. ☐ Appear in person. All ages. No exceptions for children. ☐ Present valid Missouri State Driver License or valid government ID. ☐ Present proof of U.S. Citizenship (original birth certificate, U.S. Passport or original naturalization certificate). ☐ Provide one passport photo. Passport photo services are available at Library passport acceptance locations for a $15 fee. ☐ Pay two separate fees to the U.S. Dept. of State and to the Library. If paying by check, two checks are required. ☐ Pay the required fees to the U.S. Dept. of State (check or money order only). Families may write one check or money order to the U.S. Department of State for their passport application fees if they are applying in person at the same time. For all other applicants, separate checks must be made for each application. See Fees section. ☐ Pay the required fees to the Library.
